　はじめに

　先天性頸耳瘻孔または嚢胞は，胎生3週から6週目に現れる第一鰓溝の発育異常・遺残から生じる。今回，比較的稀な第一鰓溝性瘻孔を経験したので，その臨床的特徴，病理組織像，顔面神経との関連などにつぎ若干の文献的考察を加え報告する。

First branchial cleft fistula was found in a one-year-old female. She complained of a painful swelling and a discharge from an opening below the left angle of the mandible, in addition, a drainage from the left external auditory canal. A fistulous tract connecting these draining points, which were located medially to the facial nerve trank, was confirmed by the surgery.

For diagnosis, it is important to pay attention to this fistula and not to attempt mutiple incisions which make a curative surgery much more difficult.
