　緒言

　上顎癌は顔面という整容上最も重要な部位に発生するため，従来，十分な切除がためらわれる傾向にあった^1,2），しかし，近年の治療において微小血管吻合を用いた遊離複合組織移植の果たす役割は多大なものとなっており，拡大切除により失われる形態および機能の再建を飛躍的に向上させた^3,4）。

　われわれは，それら再建の一環として，通常閉鎖されている患側鼻腔を，残存する鼻中隔粘膜と遊離組織の皮膚面で形成することを試みてきた。今回，鼻腔形成を行った4例と行わなかった3例に関し，同術式の利点・問題点を検討したので報告する。

In this paper, seven patients who underwent maxillectomy are presented. Four of them (Group A) received plasty of the nasal cavity with skin surface of a free myocutaneous flap and remaini-ng nasal mucosa. Closure of the nasal cavity was performed in other three patients (Group B). In Group A, the air passage through the reconstru-cted nasal cavity was observed in two patients and all patients complained of nose wax. In Group B, a feeling of nasal obstruction still bo-thered all patients.

We conclude that 1) the plasty of the nasal cavity is advisable for patients whose nasal cavities are obstructed and for younger patients. 2) One of the main causes of postoperatives stenosis is thought to be pushing by the flap. 3) Despite postoperative stenosis, our method should be im-proved for several reasons.
