鼓室形成術後の鼓膜再穿孔について

Grafting Failure after Tympanoplasty

　はじめに

　鼓室形成術における鼓膜形成は，聴力改善とともに鼓室形成術の最も大きな目的の一つである。したがって，その再穿孔をいかに少なくするかについては大きな努力がはらわれるべきである。しかし，実際の鼓膜形成後の再穿孔率は，鼓膜材料として筋膜が用いられるようになってからでも5〜10％程度とする報告が多い。むろん，再穿孔を少なくするために色々な工夫もなされている。

　われわれの施設では，しかし再穿孔予防のための特別な方法を用いずに，その率を比較的低率におさえることができている。そこで，過去11年4ヵ月間に行った鼓室形成術症例について，鼓膜形成の方法を述べるとともに，再穿孔例について検討を加えた。

The results of grafting in 579 ears receiving primary tympanoplasty during 1979 to 1990 were analyzed. The tympanic membrane reconstruction was done using inlay technique. Postoperatively permanent perforation was obseved in 4 ears (0.7 %) and the transient perforation during the ob-servation period was observed in 6 ears (1.0%). One of the most influencing factors which bring good grafting results is the operating procedure to the tympanic membrane under the direct and clear vision.
