唾液腺腫瘍の2症例—眼窩内進展例と副咽頭間隙進展例

Salivary Gland Tumors Extended into the Orbit and Parapharyngeal Space:Report of Two Cases

　はじめに

　唾液腺原発種瘍は口腔粘膜の小唾液腺由来と，耳下腺，顎下腺および舌下腺の大唾液腺由来とに大別される。本報告は口腔の頬粘膜小唾液腺原発と推定した腺癌の1症例と，耳下腺の腺様嚢胞癌で5回の再発をみた1症例の症例報告であるが，2症例は共通して側頭下窩，側頭窩への進展をきたし，前者には眼窩内進展，後者には副咽頭間隙進展が加わるものである。2症例の腫瘍の進展経路および画像診断上の特徴について，文献的考察とともに検討する。

In case 1, adenocarcinoma originated at the buccal mucosa in the right parotid gland. Exten-ded into the infratemporal fossa, the temporal fossa and the right orbit via the inferior orbital fissure.

In case 2, a 50-year-old female, adenoid cystic carcinoma originated in the right parotid gland. Recurrence has occured five times during the last 18 years, but all were treated surgically with some success.

The latest recurrence was observed at the tem-poral and infratemporal fossae, parapharyngeal space up to the level of foramen ovale.

A rather extensive surgery was done wigh a MC flap from the chest wall.
