顔面神経麻痺を合併した小児の結核性中耳炎症例

Infantile Tuberculous Otitis Media with Facial Nerve Paralysis

　はじめに

　近年の予防医学ならびに化学療法の進歩により結核性疾患に遭遇することは稀となったが，今なお臨床医にとっては重要な疾患である。しかし本疾患は稀であるがゆえにしばしば見過ごされたり，あるいは他の疾患と誤まって治療を受けていることも少なくない。

　今回，われわれは他院にて長期間にわたり中耳炎の加療を受けていたにもかかわらず，顔面神経麻痺を合併しその後に結核性中耳炎と判明した小児の1症例を経験したので，その概要を報告するとともに本症の診断および治療に関し若干の文献的考察を加える。

A 4-year-old girl presented with complaints of otorrhea which was not responded to previous antibiotic treatments, and the patient recently developed facial paralysis. The otoscopic exami-nation revealed a whitish granulation in the mid-dle ear. Suspicion on tuberculous otitis media prompted a smear staining of discharge and tuber-culin skin test with positive results. The antitu-berculous treatment was initiated immediately. A month later a tympanomastoidectomy was supplemented. The discharge was controlled and the paralysis was cured quickly.

Although infrequent, possible tuberculous otitis media must be remembered when seeing uncon-trollable otitis media with usual antibiotic treat-ments.
