頭頸部手術におけるフィブリン糊の応用

Clinical Application of Fibrin Tissue Glue in Head and Neck Surgery

　はじめに

　近年，ヒト濃縮フィブリン糊が商品化され耳鼻咽喉科領域では耳科手術^1〜9），鼻出血や鼻内手術^1,8,10），ガマ腫^11），口蓋扁桃摘出術^12）などへの応用が増えつつある。このうち特に耳小骨など骨組織からなる中耳の手術でのフィブリン糊の有用性が高く，数多くの報告がなされている^1〜9）。しかし，頭頸部領域でのフィブリン糊の使用経験の報告はそれほど多くはない^7）。

　頭頸部手術の問題の1つに顎顔面や頭蓋底部の露出骨の処理の問題がある。上顎腫瘍摘出後の露出骨創面の治癒遷延や腐骨，また，頭蓋底手術に伴う髄液漏などが厄介な術後合併症としてしばしば問題になる。われわれは上記の頭頸部手術の骨部術創にフィブリン糊を応用し良好な結果を得たのでその有用性について報告する。

We evaluated clinical application of a fibrin glue (Beriplast®-P) in head and neck surgery. This glue is very useful for preventing a cere-brospinal fluid leak with a muscle flap in skull base surgery (3 cases). Stabilization and adhesion of a grafted mesh split thickness skin flap on a raw surface of surgical wound was carried out using this glue in four cases with partial maxil-lectomy. This glue is very convenient and easy for adhesion of a free flap on the raw surface of bone, and then epithelization was achieved within three weeks after operation without complications.
