　はじめに

　わが国では，それまで順調に減少していた結核の罹患率が1997年に増加に転じた。集団感染や院内感染の報告が相次ぐ中で，つつに1999年7月に厚生省が結核非常事態宣言を出すに至った。また，INH（iso-nicotinic acid hydrazide）とRFP（rifampicin）に対して同時に耐性を示し，通常の抗結核薬治療では治療困難な，いわゆる多剤耐性結核が報告され，新たな対策が必要とされている。

　今回，われわれはRFP耐性結核菌による肺結核の患者で，呼吸器症状の発症する以前に耳症状が出現した症例を経験した。診断に苦慮したが，最終的には結核性中耳炎と確定診断して，保存的方法で耳漏を制御し得たので，若干の文献的考察を加えて報告する。

A 35-year-old man had chief complaints of left otorrhea and hearing loss. A large central perfora-tion with granulation tissue was observed. Tubercu-lous otitis media was diagnosed from a PCR assay and a histopathological examination of granulation tissue by Ziel-Neelsen stain, despite several nega-tive results of smear and culture tests of the speci-mens of his otorrhea. He was administered both anti-tuberculosis chemotherapy (INH, SM, PZA. EB), and ear drops of OFLX, FOM and KM. The treatments were effective in this case.
