　はじめに

　近年，ABRやMRIの普及によって聴神経腫瘍の診断は容易になり，聴力障害の軽度なうちに診断される機会が多くなってきており，これに伴い術後聴力保存例や聴力改善例の報告も多くなってきた。一方，聴神経腫瘍の早期診断におけるABRの診断的意義については多くの報告があるが，ABR所見の変動や術後のABR所見の改善についての報告はほとんどない。

　われわれは1988年以降，比較的早期の聴神経腫瘍7例に対して脳外科の協力のもとに中頭蓋窩法摘出術を施行してきたが，そのうち5例は術前会話音域聴力レベルが30 dB以内であり，聴力保存を試みた。うち3例において術前と同じ聴力を保存して摘出し得たが，これら3例全例において術後にABRの正常化を認めたので報告し，聴神経腫瘍におけるABR所見の変動について考察する。

Three cases of acoustic neuroma excised with postoperative preservation of cochlear nerve func-tion and normalization of auditory brainstem response (ABR) are reported. All three cases had a nearly normal hearing preoperatively and underwent surgery by the middle fossa approach. Whereas improvement of ABR findings was ob-served within one week after surgery in case 3,ABR were impaired about two weeks after sur-gery and normalization of ABR was observed within 7 to 10 months following surgery in cases 1 and 2.

The mechanisms of improvement in ABR find-ings after acoustic neuroma surgery arc discuss-ed and the literature concerning improvement and normalization of ABR in patients with acoustic neuroma and after acoustic neuroma surgery was reviewed.
