A 67-year-old female underwent total resection of left acoustic neurinoma through the retrolaby-rinthine approach. Before the operation she had moderate sensorineural hearing impairment and ABR had a normal Ⅰ~Ⅴ interval. Twelve days after the operation ABR was not demon-strated and there was severe hearing impairment of mixed type, suggesting middle ear effusion and inner ear disturbance. Although the letency of each wave was slightly delayed, ABR was demon-strated again 15 days after the operation. ABR then recovered to the preoperative level one month after the surgery except letency of I wave wasdelayed. Although air-bone gap disappeard, hear-ing level of bone conduction deteriorated, sug-gesting permanent inner ear disturbance. Per-manent inner ear disturbance after the surgerymight be due to edema of the cochlear nerve, compression of the nerve by clot, or disturbance of blood supply as a result of the surgical pro-cedure.
