嗄声を主訴としたWerner症候群の2例

Two Cases of Werner's Syndrome with Hoarseness

　はじめに

　1904年にキール大学の眼科医，Werner^1）は，早期老化現象を主徴とする，強皮症様皮膚変化などを合併した若年性白内障の症例を発表した。のちに1934年になって，Oppenheimerら^2）がこれらの症候に骨粗鬆症，高血糖症，生殖器機能不全などの症候を加えWerner症候群の名を冠した報告を行った。さらに1945年にはThannhauser^3）により，また，1953年にはIrwinら^4）によってその主要徴候が詳細にまとめられた。このなかには高調嗄声（weak and high-pitched voice）が含まれているが，これは全身疾患であるWerner症候群の一部分症にすぎず，他科領域の症状が初発症状となることが多いため，患者が最初に耳鼻咽喉科を受診することは稀であると考えられる。

　Werner症候群は本邦でもすでに200例あまりが報告されているが，最近では老化現象解明の手掛かりとして注目されており，DNAレベルでの研究^5,6），免疫能からの研究^7）などが報告されている。

Two patien's presnted with dermatological signsas the initial symptom, and they visited us for the evaluation of weak and high pitched voice. They also showed middle ear lesions on x-ray examinations of the temporal bone.

The pitch of voice in one patient ranged from 160 to 220 Hz, and it appeared a litlle higher than that of normal adult male.

It was speculated that hoarseness of Werner's syndrome is due to tissue changes in the vocal cord due to abnormality of hyaluronic acid meta-bolism.
