顎下腺摘出後の味覚性発汗症例

Gustatory Sweating Following Submandibular Salivary Gland Removal

　はじめに

　味覚性発汗は一般的に耳下腺の手術や外傷の後でみられ，フライ症候群として知られている。摂食時に耳介前部と側頭部の発汗と紅潮，不快感を伴う。症状発現時期は，耳下腺の手術や外傷後数カ月より数年と種々であり，更にその原因として神経再生における皮膚神経と唾液腺支配の自律神経との誤短絡が考えられている。この味覚的発汗は普通耳下腺領域にみられ，同じ大唾液腺でも顎下腺領域にみられることは極めて稀で，本邦での報告はみあたらない。

　今回私達は顎下腺摘出後に同側顎下部皮膚に限局した味覚性発汗を経験したので，若手の文献的考察を加え報告する。

A case of gustatory sweating overlying the right body of the mandible following excision of the right submandibular salivary gland is reported. The etiology appears similar to that of the auriculotemporal syndrome, but the efferent nerve effecting the reflex is concluded to be the chorda tympani nerve rather than the auriculotemporal nerve.

Treatment with interposition of Tutoplast Dura was found to be effective in reducing the symptoms.
