A 36-year-old male with vertigo was examined with a suspicion of peripheral labyrinthine disor-der. However nystagmus test results suggested central nervous system (CNS) involvement, and CT scar revealed a metastatic brain tumor, originating from lung adenocarcinoma. The patient died 3 months after the first examination.

Vertigo, caused by CNS impairment, frequently lacks typical CNS-related clinical signs in the early stage. Therefore, patients complaining of vertigo should be carefully examined by neuro-otological tests (including nystagmus test), taking into consideration the possibility of CNS diseases.
