閉経後に退縮傾向を示した頸部Extra-abdominal desmoid tumorの1例

Extra-abdominal Desmoid Tumor of the Neck Regressive in Post-menopausal Status: Report of a Case

緒言

　Extra-abdomillal desmoid tumorは病理組織学的には良性像を示すが，その腫瘍の性格は旺盛な浸潤性増殖を特徴とし，しばしば局所再発を繰り返すなど臨床的に悪性を思わせる経過をとることが知られている。他方desmcid tumorの発育に性ホルモンが関与することはすでにMusgrove^1），Pack＆Erhlich^14）などの報告に述べられているが詳細については不明の点も多い。

　今回著者らは頸部に発症したextra-abdominaldesmoid tumorの1症例を経験した。本症例は計4回におよぶ切除術と再発を繰り返すなど著明な増殖力を示し，制御には困難を極めたが，閉経を境に腫瘍の増殖が緩慢となり部分的に退縮傾向も認められた。desmoid tumorと性ホルモンの関連を支持する貴重な症例と考え報告する。

A 44-year-old woman developed a tumor of the neck which had remarkably proliferative potency. Resection of the tumor was performed 4 times in 8 years, because of reproliferations in each occa-sion.

In postmenopausal status, however, the proli-feration of the tumor was abated, and even spon-taneous partial regression was observed.

This case suggests that the growth of extra-abdominal desmoid tumor is depending on sexual hormone, especially estrogen.
