骨性外耳道閉鎖に伴う耳性脳膿瘍の側頭骨病理

Temporal Bone Histopathology of Otogenic Brain Abscess with a Bony Closure of the External Canal

はじめに

　化学療法の進歩によって耳性頭蓋内合併症は著しく減少し，耳性脳膿瘍に遭遇することは稀になった。今回われわれは原因不明の骨部外耳道の異常増殖から始まり，耳性脳膿瘍に発展して死亡した1例のCTならびに側頭骨病理所見について検討したので報告する。

Temporal bone pathology in a 17-year old man who died of an otogenic brain abscess following bony closure of the external canal was reported. It was found that cholesteatoma filled the atic and antrum, and the bony plate of the middle cranial fossa and the posterior cranial fossa were destructed. The cause of the brain abscess was considerd to be: 1) loss of the drainage route due to the bony closure of the external canal, 2) weakness of the temporal bone stracture due to ossifying fibroma, 3) destructin of temporal bone duc to choles-teatoma. This study of the temporal bone pa-thology indicates that closure of the external canal is most likely the cause of intracranial complications.
