呼吸障害を呈した小児喉頭囊胞の1例

A case of pediatric laryngeal cyst accompanied by respiratory disorder

はじめに

　乳幼児における喉頭囊胞は，稀ではあるが呼吸困難や嚥下障害などをきたし，診断・治療の遅れは死に至る可能性もある疾患である^1〜3）。小児先天性喉頭喘鳴の原因疾患として，喉頭軟化症が32％，声帯麻痺が22％，喉頭狭窄が24％，喉頭血管腫・乳頭腫などの腫瘍性疾患が11％，囊胞性疾患が7％との報告がある^4）。新生児や乳幼児の喘鳴を伴う呼吸障害では，頻度は少ないものの喉頭囊胞の可能性も常に念頭に置いて診療にあたる必要がある。出生時から重篤な呼吸障害を発症する例もあるが，感染などを契機として急激に呼吸障害を発症する例もあるので注意が必要である。

　しばしば診断に難渋することもあり，診断，気道確保，外科的治療の適応などを迅速かつ的確に判断することが重要である。そのため耳鼻咽喉科だけではなく，小児科，麻酔科など複数科での協力が必要である。今回われわれは，囊胞穿刺減量術を併用した囊胞開窓術で良好な経過を得た小児喉頭囊胞の1例を経験したので，若干の文献的考察を加えて報告する。

　A two-year-old girl with stridor and frequent apnea visited our hospital. The patient was diagnosed with laryngeal cyst accompanied by a respiratory disorder. Laryngoscopic marsupialization was performed after puncture deflation of the cyst. Her stridor disappeared after the surgery. Based on the site of origin, size, and relationship with the laryngeal mucosa, it was determined to be a lateral saccular cyst. Laryngeal cysts are often difficult to diagnose in children. Rapid and accurate responses, including diagnosis, airway management, and consideration of the indications for surgical treatment, are therefore important. Cooperation is necessary among multiple departments, including otorhinolaryngology, pediatrics, and anesthesiology.
