生体内吸収性ミニプレートによる頰骨骨折の観血的治療

Fixation of the zygomatic fractures using bioabsorbable bone fixation plates

はじめに

　頰骨骨折を含む顔面骨骨折は，頭頸部に限局した比較的軽度の外傷から，交通事故や転落などの高エネルギー外傷である全身的外傷に随伴して認められる場合まできわめて多彩である。頭頸部領域には中枢神経系に加えて聴覚，嗅覚，視覚，味覚，触覚などの日常生活に特に重要な感覚機能が集中している。また，顔面は審美的な要素も併せもつため，顔面機能と形態に著しい障害を残す可能性のある顔面骨骨折は機能面・整容面の両面からのアプローチが求められる。近年，頰骨骨折を含む顔面骨骨折の整復材料に，従来の金属製プレートに代わって生体内吸収性ミニプレートが応用されるようになってきた。今回われわれは，頰骨骨折と診断され整復後の固定に生体内吸収性ミニプレートを使用した症例を，整復のみで内固定を施行しなかった症例と比較し，臨床的な検討を行った。症例を呈示し，文献的な考察を加えて報告する。

　Bioabsorbable bone fixation plates have a distinct advantage over metal plates, as they need not be surgically removed. Here we report clinical outcomes of 10 patients with traumatic zygomatic fractures；seven of them underwent internal fixation using bioabsorbable plating system, and the other three underwent only open reduction without fixation. Four patients, including the three without internal fixation, developed postoperative re-dislocation of the reduced malar bone, suggesting the necessity of internal fixation for dislocated malar bone. Bioabsorbable plates would help lower our psychological barrier for surgical malar bone fixation even in patients with minimally-dislocated zygomatic fracture.
