はじめに

　梅毒はTreponema pallidum（Tp）による全身性の性感染症である。本邦での報告数は2011年から増加に転じており，2016年には日本性感染症学会から注意喚起が通達されている。近年は性交様式の多様化に伴い，口腔咽頭症状を初発症状とする梅毒患者が増加傾向にある。耳鼻咽喉科領域の代表的な梅毒症状として口唇の硬結や軟口蓋の白斑（butterfly appearance）が挙げられるが，初診の段階では他疾患との鑑別が困難なことも多い。今回われわれは，頸部リンパ節腫脹を主訴に来院し，下咽頭に粘膜疹を認めた咽頭梅毒の1例を経験したので，文献的考察を加えて報告する。

　A 51-year-old man presented with enanthem involving the hypopharynx and bilateral cervical lymphadenopathy. He showed an acute disease course with suspected inflammatory changes, and antibiotics were prescribed. However, he showed no improvement, and biopsy evaluation of a specimen obtained from the hypopharynx under general anesthesia suggested the possibility of a malignant tumor. We observed no obvious findings of malignancy, and preoperative examination showed a positive serum syphilis reaction, leading to a diagnosis of syphilis. Syphilis typically presents with enanthem involving the lips and oral cavity；however, it should be considered in the differential diagnosis in patients in whom enanthem is observed in the hypopharynx.
