はじめに

　血瘤腫は臨床上の診断名であり，出血像や凝血塊を主体とした腫瘤性病変の総称で，良性疾患である。詳細な成因や病態は解明されていないが，抗凝固薬や抗血小板薬の服用などによる出血傾向が血瘤腫形成に関与する可能性が示唆されている。片側性鼻出血を反復する症例，あるいは画像検査にて骨びらんや骨破壊が認められ悪性腫瘍やアレルギー性真菌性副鼻腔炎などが疑われる症例については，鑑別疾患として本疾患も念頭に置く必要がある^1）。

　鼻副鼻腔血瘤腫の治療法は基本的に外科的切除であり，内視鏡手術や外切開による摘出が行われる。また術前に，出血のコントロールを目的として栄養血管に対して塞栓術が施行されることもある^2）。

　今回われわれは，当院で経験した鼻副鼻腔血瘤腫3例について，若干の文献的考察を加えて報告する。

　Organized hematoceles are benign hemorrhagic masses. We report three cases of nasal and paranasal sinus organized hematocele. The predominant occurrence site was the maxillary sinus in two cases, and the sphenoid sinus in one case. Angiography was conducted in two cases arising in the sphenoid sinus and the maxillary sinus, but it did not reveal the feeder artery. In all 3 cases, we were able to perform successfully curative treatment via endoscopic sinus surgery. For complete surgical resection of organized hematoma of the sphenoid sinus, we should choose the wide sphenoidotomy including the detection of foramen rotundum and pterygoid canal.
