内視鏡下副鼻腔手術を施行した好酸球性副鼻腔炎症例の検討

Clinical study of eosinophilic chronic rhinosinusitis treated by endoscopic sinus surgery

はじめに

　慢性副鼻腔炎に対する治療は1990年代以降に，マクロライド療法の一般化と内視鏡下副鼻腔手術（ESS）の普及により大きく変貌した。しかしながら，症例を重ねるうちにこれらの治療に抵抗する難治性副鼻腔炎に遭遇するようになった。難治性副鼻腔炎はいくつかのフェノタイプに分けられるが，その代表の1つが好酸球性副鼻腔炎（ECRS）であり，病態機序の解明や診断基準の検討，あるいは治療方針に関するコンセンサスが検討されている^1）。ECRSに対する手術治療の効果については多数報告されている^1,2,3）。

　今回われわれは，保存的治療にて十分な治療効果を認めず，ESSを施行した好酸球性副鼻腔炎症例の術前後の自覚症状，QOLの変化，また，内視鏡所見の変化について検討を行った。

　Eosinophilic chronic rhinosinusitis（ECRS）is a subgroup of chronic rhinosinusitis（CRS）with nasal polyps, and a refractory disease. We conducted questionnaire surveys to assess the subjective symptoms and evaluate the quality of life（QOL）, and evaluated endoscopic nasal polyp score before and after endoscopic sinus surgery. All subgroups of ECRS improved most of subjective symptoms including sense of smell, and QOL scores after surgery. We submitted that not only the way of ESS but also post-surgical medical treatment are important because ECRS tends to recur.
