さまざまな経過をたどった再発性多発軟骨炎の3例

Three cases of relapsing polychondritis

はじめに

　再発性多発軟骨炎relapsing polychondritis（RP）は，全身の軟骨組織特異的に慢性かつ再発性の炎症をきたす疾患である。本邦では2015年より厚生労働省の指定難病に認定されている^1）。診断基準としてDamianiらのもの（表1）^2,3）が汎用されている。現在でも1割程度の死亡例が存在し，その約半数は気道軟骨炎による呼吸器関連が原因と報告されている。今回，当科で①耳介病変のみ，②喉頭病変に対して免疫抑制療法中，③喉頭病変に対して気管切開後の3症例を経験したので，文献的考察を加えて報告する。

　Relapsing polychondritis is a chronic inflammatory disease in cartilage tissue. The primary cause of death by this disease is respiratory failure. We report three cases of relapsing polychondritis. The predominant occurrence site was the auricular cartilage in one case, the auricle and airway in another, and the airway without auricles in the last case, which underwent tracheostomy. In the second case, after the diagnosis of relapsing polychondritis, continuous observation was performed by a physician. The patient used immunosuppressants to avoid suffocation, and his medication prevented tracheostomy in this case. Otolaryngologists should cooperate with physicians to manage relapsing polychondritis.
