はじめに

　慢性副鼻腔炎（chronic rhinosinusitis：CRS）は耳鼻咽喉科医が対応する疾患のなかでも頻度が高く，保存的治療に抵抗する症例では内視鏡下鼻副鼻腔手術（endoscopic sinus surgery：ESS）が選択される。近年ではマクロライド少量長期投与が無効な好酸球性副鼻腔炎（eosinophilic CRS：eCRS）の比率が高くなっており，手術が必要となる症例が増えてきている。特にeCRS症例では炎症が強く，また鼻洗浄など長期の術後管理を想定してⅣ型ESSを選択されることも多いことから，出血のコントロールや術後の創部管理も重要である。術後の鼻内パッキングの意義として，術後出血の予防と創傷治癒の促進が挙げられるが，圧迫感や痛みなどの不快感や感染が問題となる。以前は抗菌薬含有軟膏を塗布したガーゼを挿入することが一般的であったが，生体分解性や吸収性の素材が開発され，上記の問題が解決されつつある。当院ではESS後の鼻内パッキングにナゾポア^®（日本ストライカー社）を使用しており，その安全性と有効性を検討した。

　Nasal packing after endoscopic sinus surgery（ESS）for chronic rhinosinusitis contributes to less postoperative bleeding and better wound healing. NasoPore is a biodegradable synthetic polyurethane foam that has recently been used often in ESS. Patients treated with NasoPore packing were reviewed, and its safety and usefulness were evaluated. Of the 46 patients, one experienced bleeding requiring extended hospitalization. No severe infections were observed. Although transient edema was seen in eight patients, crust formation, adhesions, and polyp formation were observed in only a few. NasoPore is effective for avoiding postoperative bleeding and improving wound healing.
