耳下腺部に発生したCastleman lymphomaの1例

Castleman Lymphoma of the Parotid Region: A Report of Case

I．はじめに

　1954年Castieman^1）が縦隔内に発生した胸腺類似の腫瘤を"lymph node hyperplasia"として記載して以来，同様の症例は220例以上，本邦でも60例以上が報告^2）されており，一般にCastlemanlymphomaと呼ばれている。発生部位では縦隔以外にも頸部，腹部，腋窩，鼠径部，その他さまざまな例が知られている。

　私どもは7,8年間に徐々に増大した耳下腺部腫瘤を呈した本症（hyaline-vascular type）を経験した。本症が耳下腺部腫瘤として発生する例は比較的少なく，また手術時に顔面神経の取扱い方の問題もあったことから，本症例を報告し文献的考察を加えた。

A 43-year-old man, presenting an egg-sized mass in the right parotid region, was reported.

The mass had gradually increased in size for over seven years without symptoms.

Removal of the tumor was performed under general anesthesia preserving the continuity of the facial nerve, whose branches were found to be involved in the tumor.

Histological examination revealed Castleman lymphoma.
