Ⅰ　はじめに

　第二鰓裂由来の完全型側頸瘻は頸部では胸鎖乳突筋の下1/3前縁に，口腔側では口蓋扁桃にそれぞれ開口する。治療の原則は外科的摘出術である。一般に，頸動脈・静脈そして神経系の損傷を避けるために前頸部を切開して直視下に瘻管のみを剝離摘出する^1）。症例によっては瘻孔開口部以外に追加の切開が必要である^1）。

　筆者は既に瘻管内に通したナイロン糸の口腔側に小ガーゼ球を結紮して頸部側から牽引しつつ瘻管のみを摘出する術式を発表した^2～4）。今回，小さい口腔の乳幼児において，中空のbiopsy punchを用い口腔内手術操作を簡便とする術式を考案したので報告する。

　Under general anesthesia insertion of a nylon thread through a fistula was done. On the oral site of it,a small gauze ball was tied and was pulled from the neck site. The length of fistula shortened. At the neck sites of the fistula,a small incision around the nylon thread was performed. Using the nylon thread as a guide,almost of fistula was removed. On the neck side of nylon thread,a new gauze ball was tied and pulled from oral side. A remaining oral site of the fistula was removed using a biopsy punch for ceps and was closed with absorbable thread.
