両側先天性完全側頸瘻孔症例

CONGENITAL CERVICAL FISTULA

　I．はじめに

　先天性側頸瘻孔は主に胎生期における鰓性器官から発生する先天性奇型と考えられている。側頸瘻孔が第1鰓裂性器官から第4鰓裂性器官までのうち，いずれの鰓裂に由来するかは，瘻管の開口部の位置，瘻管の走行とその周囲組織との関係によつて決定することができるといわれている。しかし，完全瘻孔を除いてはその由来を決定することは困難な場合が多いようである。

　最近われわれは4歳の男子で両耳瘻孔を合併した第2鰓裂由来による両側完全側頸瘻孔を経験したのでここに報告した。

　A bilateral complete cervical fistula which originated from the combination of aural fistula and the ramnant of 2nd branchial cleft in a boy, aged 4, is reported.

　Reports on cases of this nature is very rare in the literature of this country ; only 2 cases are found so far. The diagnosis was easily made by instillation of radio opaque material. The surgical removal of the fistula was greatly aided by passage of nylon suture into its opening.

　The lining of the fistula was studied under microscopic examination.
