Ⅰ　はじめに

　口腔，咽頭異物は日常診療でしばしば経験する疾患だが，そのほとんどは口腔や咽頭の浅層に留まる異物であり，外来で摘出可能なことが多い。その一方，箸などの異物が口腔内より刺入し脳脊髄や大血管の損傷をきたして重篤となる症例も存在する^1～3）。このような症例では，異物の存在位置に対する画像診断の重要性は述べるまでもないが，摘出前後の気道確保や大血管損傷に伴う出血や血栓への対応，他科との連携なども必要となってくる。今回われわれは，口腔内より刺入した箸が副咽頭間隙を経て内頸動脈の深層を進み後頸部に停止していた症例を経験したので報告する。

　An interesting case of a wooden foreign body in the neck entering through the right side of the oral floor was presented. The foreign body traveled subcutaneously in the neck without damaging any vital structures. The wooden foreign body showed a very low density almost same as that of air in the enhanced CT scanning,therefore,it was impossible to distinguish the wooden foreign body from air. Reconstructed enhanced CT image concerning with the density of the foreign body to distinguish from air was very useful to understand the exact size and position of the foreign body,and the relationship between the foreign body and vital structures especially the internal carotid artery.
