抗甲状腺薬による血小板減少が疑われ手術を要した小児バセドウ病の1例

An operated pediatric Basedow disease with thrombocytopenia due to anti thyroid drug：a case report

はじめに

　バセドウ病は，甲状腺のびまん性腫大を有する後天性自己免疫性甲状腺機能亢進症であり，小児期発症例では薬物治療が第一選択となる。しかしその寛解率は約30％と成人に比して低く，治療に難渋することも少なくない。今回われわれは抗甲状腺薬の副作用として血小板減少が疑われ，手術加療を行った1例を検討した。甲状腺機能のコントロールや出血のリスクなど多くの課題を認めたが，小児科，内科と共診のうえ，可能な限り予測されるリスクを回避して手術に臨むことができた。本邦において手術にまで至った小児バセドウ病の報告は少なく，検討すべき事案も多い。本症例を通して得られた課題をまとめ，文献的考察を含め報告する。

　The major treatment of pediatric Basedow disease is medication, but it is also difficultd to get remission. A 10-year-old girl, who developed resistance to anti-thyroid drug due to thrombocytopenia. We finally performed total thyroidectomy in collaboration with pediatricians and physicians for managing thyroid hormone and platelet by using with gamma-globurin. It was considered that we need to cooperate with specialists of other field for avoiding any risk.
