Ⅰ　はじめに

　頸部壊死性筋膜炎は筋膜や筋肉のみならず皮膚や皮下組織へも急速に出血性壊死性病変が拡大する重篤な病態である^1）。下降性に縦隔まで炎症が及ぶと縦隔膿瘍を形成して治療に難渋することもあるので，早急かつ適切な治療が必要である。特に高齢者では，全身性疾患の合併症を伴う例が多く，急激な経過をとる場合や，また自覚症状が乏しく訴えが少ないために診断と治療が遅れて死に至る場合があるので，注意が必要である。

　今回われわれは，頸部壊死性筋膜炎から深頸部膿瘍と縦隔膿瘍を形成し，治療に難渋した高齢の症例を経験したので，文献的考察を含めて報告する。

　The cervical necrotizing fasciitis（CNF）is a grave infectious disease and easily induces the mediastinitis. Its progress is very severe and mortality is high. We reported a case of an 85-year-old woman suffering from the CNF with mediastinal abscess. She was treated with surgical debridement and drainage in cervical necrotizing fasciitis and mediastinum abscess and intravenous antibiotic agents. Severe dysphagia occurred by post-inflammatory scar formation of the neck but finally healed by rehabilitation. Active treatment with both surgery and medication is crucial for overcoming the life-threatening CNF.
