Ⅰ．はじめに

　頭頸部進行癌に対するシスプラチン（CDDP）を用いた超選択的動注化学療法（以下，動注と略す）は，1992年Robbinsら^1）によって紹介され，その優れた臓器温存率からわが国でも普及しつつある^2）。動注は放射線治療と併用されるのが一般的であるが^2,3），今回，手術治療を強く拒否されたため，以前の放射線照射野に発生した舌根癌症例に対して，動注単独で原発巣を治療する機会を得たので，治療経過について報告する。

　Superselective intraarterial chemotherapy（IAC）is an excellent therapy against head and neck carcinomas on account of the high rate of organ preservation. A 72-year-old male received radiation therapy against supraglottic laryngeal carcinoma（T2N0M0）ten years ago,presented with pharyngeal pain and bilateral cervical swelling. The examination revealed mesopharyngeal carcinoma arising in the tongue base（T4aN2cM0）. Because he refused surgical operation of the primary site,he received IAC without concomitant radiation therapy. The primary site resulted in CR,and he underwent bilateral neck dissection against remnant cervical lymph nodes.
