60歳以上の口蓋扁桃摘出例の手術効果に関する検討

Efficacy of Tonsillectomy for Cases Over 60 Years Old

I．はじめに

　一般的に高齢者の口蓋扁桃は肉眼的にも萎縮している。図1aは77歳女性の摘出扁桃の切片で萎縮は著明で，図1bの40倍拡大の病理組織にみるように，33歳のそれと比較しても明らかに二次小節は消失し活動性が低下している印象を受ける。形浦^1）も加齢に伴う扁桃組織の変化は慢性炎症の影響も加わり退縮であるとしており，高齢者における口蓋扁桃摘出（扁摘と略）の効果の有無を臨床的に検討することは意義があると思われる。

　Faucial tonsillectomy in the aged patients appears to be innocent because of its atrophic finding including histological fibrosis and decrease in the number of the germinal centers. However,tonsillectomy for aged population has never been discussed. We have investigated nine cases,4.5％ of tonsillectomies for the past 5 years,ranging in ages from 60 to 77 years （average,67 years）. We recognized significant improvement of their symptoms in four out of 7 focal infection cases,one habitual angina case and 2 hypertrophy cases.

　This clinical results reveal that tonsillectomy should be performed actively despite of the patients' age if indicated.
