I．はじめに

　頸動脈小体腫瘍（CBT）は比較的稀な疾患で，わが国ではこれまで150例の報告があり^1），ヨーロッパでは外科病理標本のなかで0.01％の発生率であると報告されている^2）。腫瘍は頸動脈分岐部から起こり，徐々に増大し側頸部の無痛性腫瘤として偶然に発見されることが多い。およそ10％は悪性化するとされ^3），外科的切除は標準的な治療法である。画像診断と外科的技術の進歩にもかかわらず，脳神経麻痺，脳梗塞や手術死亡は，手術切除を受ける患者のなかで10～40％に達するといわれている^4）。今回われわれは，巨大なCBTにより頸動脈合併切除を行った症例を経験したので文献的考察を加えて報告する。

　A case of huge carotid body tumor（CBT）with 20-years-history is reported. MRI showed that the external carotid artery was totally encased by the tumor,while the common carotid artery and internal carotid artery passed by very closely to the margin of the tumor. Balloon matas test was negative with more than 60mmHg of stump pressure,and the cerebral blood flow（CBF）value was sufficient,which allowed the sacrification of the common carotid artery and/or internal carotid artery without vessel graft. Preoperative embolization of the external carotid artery was performed. Total resection was performed with uneventful postoperative course.
