I．はじめに

　副咽頭間隙に発生する腫瘍のうち神経鞘腫は稀な腫瘍ではないが，特徴的な臨床症状に乏しいことが多いため，神経脱落症状を伴っていない症例では術前に診断を確定することは非常に困難である。しかし神経鞘腫であった場合，手術により神経の脱落症状を余儀なくされることも想定されるため，術前に組織型や由来神経を含めた診断を推測することは，十分なインフォームド・コンセントを行ううえで重要と考えられる。

　今回われわれは，術前に画像診断によって由来神経まで推測し得た交感神経鞘腫を経験したので本症例の病態について考察するとともに，神経鞘腫における画像診断の意義について検討し報告する。

　A 72-year-old man presented with a 1-year history of the left subauricular swelling. Radiological examinations revealed a mass in his left parapharyngeal space. Although he did not manifest any neurological signs,the mass was preoperatively estimated to be a neurogenic tumor arising from the cervical sympathetic nerve on the basis of the findings of CT scan and MR imaging. The tumor was completely excised by cervical approach,and histopathological diagnosis was schwannoma. He showed miosis and palpebroptosis postoperatively,confirming that the tumor was originated from the cervical sympathetic nerve. The topographical relationship between neurogenic tumors in the parapharyngeal space and adjacent structures was reviewed from the literature.
