Passengers in the Tokyo subway system were attacked by sarin released in the air on March 20, 1995. We treated 426 patients within five days of the attack, of whom 112 had to be hospitalized. The symptoms and signs during the acute phase included extreme miosis, hyperemia, shallow anterior chamber, subnormal electroretinogram, constriction of visual field to 20 to 30 degrees, normal VEP and decreased serums levels of cholinesterase. There was no significant correlation between eye symptoms and the serum cholinesterase. The electroretinogram normalized in most patients after instillation of mydriatic agents.
