Acute ocular symptoms and the clinical course in victims of sarin terrorism

　1995年3月20日，東京地下鉄線内で発生したサリン事件において，3月20日〜3月24日の5日間に，聖路加国際病院眼科外来を受診した314名，および3月20日に入院した112名，合計426名のサリン患者急性期の症状および所見についてまとめた。最も特徴的な他覚的所見は極端な縮瞳であり，その他，充血，浅前房，網膜電図はsubnormal，視野狭窄（求心性20°〜30°），視性誘発電位正常，血清コリンエステラーゼ低下であった。血清コリンエステラーゼ値と眼症状とは特に相関はなかった。網膜電図の異常は縮瞳によるものであった。

Passengers in the Tokyo subway system were attacked by sarin released in the air on March 20, 1995. We treated 426 patients within five days of the attack, of whom 112 had to be hospitalized. The symptoms and signs during the acute phase included extreme miosis, hyperemia, shallow anterior chamber, subnormal electroretinogram, constriction of visual field to 20 to 30 degrees, normal VEP and decreased serums levels of cholinesterase. There was no significant correlation between eye symptoms and the serum cholinesterase. The electroretinogram normalized in most patients after instillation of mydriatic agents.
