A 68-year-old non-homosexual non-haemophiliac male sought medical advice for visual impair-ment. Funduscopy showed mild vitreous opacity, white exudates mixed with hemorrhage and perivascular nodular spots in the right eye. The findings were compatible with necrotizing retinitis. He developed fever, oral candidiasis and diarrhea after hospitalization. CD4 lymphocytes in the circulating blood was markedly decreased. Test for human immunodeficiency virus, HIV, was positive and led to the final diagnosis of cytomegalovirus retinitis secondary to HIV infection. This is the first reported case in Japan in which initial retinitis led to the detection of HIV infection.
