（P1-2-42）　全身疾患に対し抗凝固療法中に網膜中心静脈閉塞症を発症した1例を経験した。症例は72歳の女性で，左眼の視力低下を自覚して来院した。検眼鏡および螢光眼底造影所見から、左眼下方2象限にわたる非虚血型網膜中心静脈閉塞症と診断した。血液検査では不整脈に対する抗凝固療法中であったため重血液凝固能はトロンボテスト36％と低値を示したが，血小板凝集能は著明に亢進していた。抗凝固剤を減量し抗血小板療法を持続したところ，虚血型への移行もなく左眼視力は0.2から0.6に改善した。網膜中心静脈閉塞症は抗凝固療法中にも生じることがあり，血小板凝集能亢進が疑われた場合には抗血小板持続療法が望ましいと考えられた。

A 72-year-old female presented with sudden visual loss in her left eye. She had been receiving peroral warfarin for ventricular fibrillation since 15 years before. Funduscopy and fluorescein angiography showed nonischemic hemicentral retinal vein occlusion in the inferior hemisphere in the affected eye. Laboratory tests showed a hypocoagulable state with thrombopenia at 36% with increased platelet aggregation activity. Continuous antiplatelet treatment with anticoagulation agents induced improvement of visual acuity from the initial 0.2 to 0.6. This case illustrates that central retinal vein occlusion may develop associated with platelet hyper-aggregation and that it may respond to antiplatelet treatments.
