（B3-2-22）　1994年1月〜1997年12月に自治医科大学眼科を受診した患者のうち，サルコイドーシスと組織診断された81例（組織診断群）と臨床所見から診断された12例（臨床診断群），および眼サルコイドーシス診断基準を満たすが，確定診断には至らなかった86例（疑診群）における眼科的所見と全身検査所見，最終観察時視力について比較検討を行った。その結果，3群間における眼所見，最終観察時視力に有意差はみられなかった。全身検査所見では，確定診断群は疑診群と比較して，血清ACE上昇と67Gaシンチ陽性の出現率が有意に高かった。また，眼所見にかかわらず，検査所見が診断基準を満たす症例では，組織陽性率が高い傾向がみられた。

We reviewed 179 cases of ocular sarcoidosis seen during the 4 years through 1997. The series comprised 81 cases in whom sarcoidosis was confirmed histologically, 12 cases with clinical manifestations only, and 86 cases who did not satisfy the diagnostic criteria. There was no differences in ocular manifestations and visual outcome among the 3 groups. Cases who were diagnosed either histologically or clinically showed higher rates of positive findings by b7Ga scanning and serum angiotensin converting enzyme than those who did not satisfy the diagnostic criteria. Independent of the number of ocular manifestations, cases with more than 3 positive systemic findings were more likely to show positive histological findings.
