抗緑内障点眼薬により難治性角膜上皮障害を生じた7症例

Seven cases of severe ocular surface disorders following topical antiglaucoma agents

　最近2年間に経験した抗緑内障点眼薬に起因すると考えられる難治性角膜上皮障害の7症例を報告した。症例の内訳は，偽類天疱瘡3例，角膜上皮障害4例であり，5例ではpalisades of Vogtが消失していた。全症例で主因と考えられる抗緑内障点眼薬を中止したが，上皮障害は長期にわたり遷延した。上皮欠損を認めた2例ではepidermal growth factorの点眼が奏効した。抗緑内障点眼薬は，難治性の角膜上皮障害を生じる場合があるため，眼表面上皮の障害の発症には十分注意しておく必要があると思われる。

We observed seven cases of severe ocular surface disorders following topical treatment for primary open angle glaucoma. The ocular surface disorders included pseudocicatricial pemphygoid 3 cases and corneal epithelial troubles 4 cases. The palisades of Vogt had disappeared in 5 cases. These lesions persisted after immediated discontinuation of antiglaucoma agents. Topical instillation of epidermal growth factor was effective in 2 eyes of persistent epithelial deffect.
