再発翼状片に対する表層自己角膜移植術

Lamellar autokeratoplasty for recurrent pterygium

　再発翼状片2例に対し，これ以上再発しないようにするため，表層自己角膜移植術（偏位法）を施行した．

　この手術法の利点は以下の通りである．

（1）翼状片により破壊されたボーマン膜を有する表層角膜が，健常なボーマン膜を有する表層角膜でおきかえられるため，翼状片の角膜への再侵入が阻止される．

（2）保存角膜を必要としないため，アイバンクを持たない施設でも手術可能である．

（3）自己角膜を用いるため，拒絶反応がおこらない．

　2例の術後経過は良好で，翼状片の角膜への再侵入は認められなかった．

Lamellar autokeratoplasty was performed for the treatment of recurrent pterygium in a 53-year-old female and a 73-year-old male. Recurrence of pter-ygium was prevented in both cases as the original affected corneal area was replaced by corneal tissue with intact Bowman membrane. As further merits of the method, no reserved cornea was necessary and the postsurgical course was free of rejection.
