　医療過疎地域（僻地）や発展途上国で新たに白内障手術を始めようとする場合，高額の医療器械を導入して手術を行うことで，自治体などの財政を圧迫しないのか，どのくらいの期間で設備投資の回収が可能なのかなどの検討も重要である。この面に関する検討の第一歩として，現在筆者らが日常行っている白内障手術にかかる費用について検討した。国内の白内障手術件数は頭打ち状態であり，眼科医の増加により，医療機関1か所あたりの手術件数は減少傾向になると思われる。現在，筆者らの施設では，超音波水晶体乳化吸引術（Phaco-emulsification and aspilation：PEA）＋眼内レンズ（Intraocular　lens：I0L）挿入術1件あたりの収益が約49.000円であり，年平均300件の白内障手術では，設備投資の回収だけでも約4.5年かかると算出された。

When medical institutions in depopulated areas or developing countries intend to initiate cataract surgery, it is important to consider whether the cost for installation of surgical equipments would cause financial problems and to estimate how long it will take to recover the capital investment. Further, the number of cataract surgery at medical institutions will tend to decrease along with the increase in the number of eye surgeons in future. We therefore evaluated the cost for routine cataract surgeries. Currently, the charge for phacoemulsification-aspiration with intraocular lens implantation averages 49,000 yen in our institution. If 300 cataract surgeries are performed per year, it will take about 4.5 years to recover the capital.
