腎尿細管障害を伴う前部ぶどう膜炎

Anterior uveitis associated with renal tubular damage

　特発性前部ぶどう膜炎93例の炎症の活動期に尿細管障害の最良のマーカーである尿中β₂—ミクログロブリンの排泄量を測定した。その結果，両眼性前部ぶどう膜炎31例中の16例（51％）にその異常高値を認めた。同時に測定した尿中微量アルブミン値も同様の傾向を示した。両眼性前部ぶどう膜炎20例の経過観察から，これら尿細管性蛋白は炎症の活動期に高値を示し，副腎皮質ステロイド薬全身投与により炎症の寛解期に有意の減少を示すことが判明した。今回の成績から前部ぶどう膜炎の中でも，特に両眼性のものでは，潜在性の腎尿細管障害を高頻度（50％強）に有することが明らかとなり，両者の相関関係が示唆された。

We measured the urinary levels of β₂-microglub-lin (β₂-MG) and microalbumin (MA) in 93 patients with anterior uveitis. The uveitis was unilateral in 62 and bilateral in 31. Data from 21 healthy individ-uals served as control. The average values for normal control were: 111.5±64.3μg/g･creatinin forβ₂-MG and 6.3±4.0mg/g･creatinin for MA.Patients with bilateral anterior uveitis showed in-creased values for β₂-MG (367±347.2μg/g･Cr)and MA (32.6±42.7mg/g･Cr) during the inflam-matory active stage. The observed increase in urinary β₂-MG and MA were related to renal tubu-lar damage with disturbed reabsorption. High levels of β₂-MG and MA were observed in about half of cases with bilateral anterior uveitis and anterior uveitis with secondary glaucoma. These values ran fairly well in parallel with the clinical course of bilateral anterior uveitis. The high values of these tubular proteins decreased to almost nor-mal levels after improvement of uveitis after sys-temic corticosteroid therapy.
