硝子体手術の最近の動向—経毛様体扁平部硝子体切除術の基本術式

Some Basic Techniques of Pars Plana Vitrectomy

まえがき

　前回筆者は本誌31巻5号に，硝子体手術の最近の進歩のうち，手術装置の現状について紹介を行つたが，今回は経毛様体扁平部硝子体切除術parsplana vitrectomyの基本的な術式について解説を行つてみたいと思う。

　今回ここでのべる，術前投薬の問題，患者への術前処置，手術装置の準備，本手術を行うときの手術顕微鏡のそなえるべき条件，強膜創部位の選択，さらに基本的な手術操作などは，より複雑な硝子体切除術，すなわち，massive periretinalproliferation（MPP）を伴つた網膜剥離，tractiondetachmentを伴つた増殖性網膜症などに対する硝子体切除を行うにも不可欠な知識である。そして，今回紹介する基本的な技法によつて，網膜静脈閉塞症，Eales病などに起因する硝子体出血，糖尿病性網膜症の単純硝子体出血，あるいは硝子体アミロイドーシスなどに対する硝子体切除術は可能である。したがつて，本稿では，まず硝子体切除術の臨床応用をはじめるときに，最低限心要な事項を紹介するということになる。

Several indispensable procedures regardingpars plana vitrectomy such as preoperative instru-ment preparations, instrumentation, preparation of intraocular irrigation, preoperative patient care, anesthesia and some basic surgical techni-ques of pars plana vitrectomy, were discribed. Using these basic techniques simple vitreous pa-thologies such as vitreous hemorrhages due to retinal vein obstruction and Eales' disease as well as vitreous opacities due to primary amyloidosis or chronic uveitis can be operated on.

Absolute necessity of earful cleaning of the used instruments was enhanced.
