白内障術後炎症の角膜温度を用いた定量的評価

Evaluation of postoperative inflammation following cataract extraction by corneal temperature

　炎症パラメーターである温度の上昇が眼内の炎症を反映するか否か，非接触型放射温度計を用いて測定し検討を加えた。

　水晶体嚢外摘出術単独または人工水晶体挿入術を施行した32人，32眼の老人性白内障患者を対象とした。術前と術後1日目，2日目，7日目，14日目，30日目に角膜中央部，鼻側，耳側球結膜，上下眼瞼の温度を測定し，他眼との温度差をパラメーターとした。測定は0.1℃の分解能をもつ非接触型放射温度計THI−300を用い，直径10mmの円形部分の平均温度を求めた。

　結果は角膜中央部では，術後1，2，7，14日目に1％の危険率で，術後30日目も5％の危険率で有意の温度上昇を認めた。球結膜は，鼻側，耳側ともに術後1，2，7日目に1％の危険率で有意の温度上昇を認めた。上眼瞼は，術後1，2，7日目に5％の危険率で，下眼瞼は術後1，7日目で1％の危険率で，14日目は5％の危険率で有意の温度上昇を認めた。

　白内障術後の炎症状態において，角膜，結膜，眼瞼に温度が上昇し，これを非接触型放射温度計で測定できた。小型で測定時間も1秒以内であるので，これを用いて眼内炎症を定量化できる可能性が考えられた。

We evaluated the postoperative inflammation by corneal temperature following extracapsular cata-ract extraction and posterior chamber intraocular lens implantation. The compact, easy and reliable radiation thermometer was used in the measure-ment of the temperature. The study was comprised of 32 senile cataract patients. We examined the patients prior to the operation and postoperatively on the 1st, 2nd, 7th, 14th and 30th day. Measure-ments were taken at the central cornea, nasal and temporal bulbar conjunctiva and inferior and supe-rior eyelid skin.

We found a significant increase in temperature in all five parts measured. At the central cornea, the temperature was 1.11±0.57 on the first day, 0.75± 0.69 on the 2nd day, 0.56±0.45 on the 7th day, 0.41± 0.56 on the 14th day (p ＜ 0.01) , and 0.24±0.45 on the 30th day (p ＜0.05) .

This thermometer was useful to evaluate the postoperative inflammation.
