Abstract 文献概要

　錐体ジストロフィー38症例を眼底所見で，1） bull's eye型，2）色素沈着を伴うびまん性色素上皮萎縮型，3）色素沈着のないびまん性色素上皮萎縮型，4）脈絡膜血管萎縮型，5）眼底所見の乏しい型，に分類し各型を電気生理学的に検討した。全例で錐体ERGは低下した。bull'seye型は半数が桿体ERGとEOGも低下したが，高度障害例はなかった。びまん性色素上皮萎縮型では色素沈着を伴ったものが桿体ERG, EOGの障害が最も強く，色素沈着のないものは軽度だった。眼底所見の乏しい型は，障害はわずかだった。錐体ジストロフィーは各病型で，同じ眼底所見のものは同じ様な電気生理学的特徴を示し，本疾患の症状，経過，予後を考える際，病型別分類は有益だった。

We subdivided 38 cases of cone dystrophy into 4 types according to fundus appearances and fluores-cein angiographic features : bull's eye type 15 cases, diffuse pigment epithelial atrophy type with or without pigment clumping 17 cases, choroidal vascular atrophy type 5 cases, and normal fundus type 0 case. Each subdivision was evaluated byelectro retinogram (ERG) and electrooculogram (EOG). All the cases showed depressed cone ERG. In other respects, each type manifested peculiar and characteristic electrophysiological features. It appeared that cone dystrophy is a syndrome which includes different clinical entities. Our present sub-division into 4 types would be of value in the inter-pretation of clinical course and prognosis.
