　最近10年間に40例42眼の網膜動脈瘤を経験し，その臨床像，治療法について検討した。男性9例，女性31例で，平均年齢は66歳であった。全身所見に，高血圧症と動脈硬化症が高頻度にみられた。初診時の眼底所見は，出血型が31眼（74％），滲出型が10眼あった。動脈瘤の数は1個のものが多かった。動脈瘤はすべて第3分枝以内の動脈に存在し，第2分枝に最も多くみられた。10眼では薬物療法で動脈瘤は自然消退した。出血吸収が遷延し血管外漏出が強い症例と，黄斑部に浮腫の及んだ症例32眼には，レーザーで動脈瘤の直接光凝固を行い，全例器質化に成功した。治療として光凝固が有効であると思われた。しかし合併症として，分枝動脈閉塞や動脈瘤からの再出血例を4例経験したので，過剰凝固にならない注意が必要である。

We reviewed a series of 42 eyes of 40 patients with retinal macroaneurysm. The series included 9 males and 31 females. The age averaged 66 years. Thirty-one cases, 78 %, were aged 60 years or over. History of systemic hypertension was present in the 73 % of the cases. Macroaneurysm was of hemorr-hagic type in 31 eyes and of exudative type in 10. Only one macroaneurysm was present in a given affected eye and was located along the second arterial branch in the majority of cases.

The macroaneurysm underwent spontaneous organization in 10 eyes. Laser photocoagulation was applied to the macroaneurysm in 32 eyes with persistent retinal hemorrhage and marked dye leak-age from the macroaneurysm. Laser treatment resulted in organization of the macroaneurysm in all the treated cases.
