網膜動脈macroaneurysmの臨床像と治療

Retinal Macroaneurysms, clinical picture and photocoagulation treatment

　最近3年間に網膜動脈に発生したmacroaneurysmを11例経験し，その臨床像，治療法について検討した。10例は高血圧症，動脈硬化症をもつ高齢者で，1例のみが46歳で慢性関節リウマチの患者であった。動脈瘤は全例乳頭近くの太い動脈に発生した。動脈瘤の破裂により眼底後極部に硝子体出血や広範囲の円盤状の濃厚な網膜出血をおこして，突然高度の視力障害を来たした症例が8例あった。硝子体出血の原因として，また老人性円盤状黄斑変性症との鑑別診断に本症の診断が重要になる。診断確定には螢光眼底造影が必須であるが，新鮮例で出血が濃厚であると出血によるmaskingのため動脈瘤が見えず，出血の吸収を待ってはじめて診断された。検眼鏡によって，動脈瘤は動脈の赤い球状の拡大としてみえた例よりも，出血巣内の小円形の黄白色斑としてみえた例が多かった。

　止血剤，出血吸収剤の投与によっても自然治癒傾向のない8例にアルゴンレーザーで動脈瘤を軽く直接凝固し，何ら合併症なく，全例平均2カ月で器質化に成功した。残る3例は凝固せずに自然治癒したが，平均8ヵ月を要したので，動脈瘤の自然治癒を待つよりも比較的早く動脈瘤の直接光凝固を行うのが治療としてすすめられる。

We observed 11 cases of retinal macroaneurysms. The age of the patients ranged from 46 to 74 years. All the cases except one had a history of systemic hypertension or arteriosclerosis. One had chronic rheumatoid arthritis.

In 8 cases, sudden visual impairment occurred by massive vitreous or retinal hemorrhages due to rupture of the aneurysms. In 5 cases, hemorrhages from ruptured aneurysms were so massive as to hinder the detection of aneurysms by opthalmo-scopy.

The presence of aneurysms were confirmed byfluorescein angiography in all the cases. They were located along a main branch of retinal artery in the posterior fundus.
