コクサッキーB4ウイルスによる散在性網脈絡膜炎の1例

Disseminated chorioretinitis in a case of coxsackie virus B4 infection

　過去に報告のないコクサッキーB₄ウイルスによる散在性網脈絡膜炎の1例を報告する。症例は11歳の男子で，持続する高熱，全身倦怠感を訴え，諸検査の結果，無菌性髄膜炎，貧血，肝機能異常，心電図の異常を指摘された。視力は良好で自覚的には無症状であったが，眼底には後極部および中間周辺部を中心に血管に沿って散在する白色病巣を認めた。病巣は全身状態の改善とともに次第に軽減し，少数の瘢痕を残す以外は消失した。真菌，ウイルスを含む細菌学的検索では，コクサッキーB₄ウイルスの抗体価のみ上昇がみられ，ウイルスの直接の分離同定はできなかったが，血中の抗体価の推移からコクサッキーB₄ウイルスの感染によるものと思われた。

An 11-year-old boy developed sustained high fever with general fatigue of 2 weeks' duration. He manifested aseptic meningitis, anemia, mild hepatic failure and abnormal electrocardiogram. We obser-ved, ophthalmoscopically, scattered white patches mainly in the midperipheral retina along the vessels in both eyes. Findings were negative for blood culture, cerebrospinal fluid and urine. Serological tests showed elevated serum antibody to coxsackie virus B₄. As the general condition improved, the chorioretinal lesions became smaller and less dis-tinct to disappear except for a few scars. The visual acuity, visual field and electroretinographic find-ings were normal throughout
