要約　目的：小児のReis-Bücklers角膜ジストロフィ（RBCD）の再発性角膜上皮びらんに対する処置として，使い捨てソフトコンタクトレンズ（DSCL）装用前に角膜を搔爬することの有用性を報告する。

症例：7歳，女児。3歳時から再発性角膜上皮びらんに対しDSCL装用にて上皮治癒を図り，不正乱視に対してはハードコンタクトレンズ装用で弱視治療を行い，5歳時には良好な視力を得ていた。今回，角膜上皮びらん再発時に細菌培養のために角膜搔爬を施行したところ，角膜上皮びらんの治癒後に同部の混濁が改善する所見が得られた。そのため，以後，角膜上皮びらん再発時の対処として，DSCL装用前にびらん底および周囲の上皮の搔爬を追加したところ，その都度，搔爬部の透明性が向上し，前眼部OCTでも沈着物の減少が確認された。

結論：小児期のRBCDでは，ボウマン層に沈着したヒアリン様の沈着物質が搔爬することにより除去できる可能性がある。角膜上皮びらん再発時には，軟膏と閉瞼で沈着物を残したまま治癒を図るより，びらん底および周囲上皮を搔爬したうえでDSCL装用にて創傷治癒を図るほうが，沈着物を減少させ電気分解など外科的加療までの期間を長く保つことができる可能性があると考える。

Abstract　Purpose：To present tips for the treatment of corneal epithelial erosion in a pediatric case of Reis-Bücklers corneal dystrophy（RBCD）.

Case：A 7-year-old girl with a family history of RBCD first presented at our hospital with recurrent bilateral corneal epithelial erosions at the age of 3. We had treated her corneal epithelial erosions using a disposable soft contact lens（DSCL）and had treated her amblyopia with hard contact lens（HCL）. She had a good visual acuity at the age of 5 years.

Findings：During the follow-up period after the age of 5 years, we had scraped corneal erosion, which was suspected to be a bacterial infection in her left eye；we found that corneal opacity improved after wound healing with corneal scraping. Thereby, we speculated that debridement might help remove the deposition in the subepithelial lesion. We added the debridement of bottom of the erosion and surrounding epithelium when the corneal epithelial erosion recurred. Consequently, the corneal transparency improved after epithelial healing. We also confirmed removal of the subepithelial deposition using optical coherence tomography（OCT）.

Conclusions：In pediatric RBCD, the subepithelial hyaline-like deposits can be possibly removed by corneal debridement. We suggest debridement of the bottom of the erosion before therapeutic DSCL wearing for treatment of recurrent corneal erosion in pediatric cases of RBCD. These tips may be effective in gaining time until surgical treatment can be undertaken.
