要約　目的：整容目的で行う人工虹彩挿入術の合併症として虹彩炎，続発緑内障，角膜内皮障害，白内障などさまざまな合併症が報告されている。今回，海外にて整容目的で両眼に人工虹彩を挿入後，両眼性の水疱性角膜症に至った症例を経験したので報告する。

症例：46歳，男性。2017年にインドで整容目的で両眼に前房留置型人工虹彩挿入術を受けた。2020年12月上旬より両眼の霧視，視力低下，眼痛が出現し，視力低下が悪化したため前医を受診した。初診時矯正視力は右0.2，左0.03，眼圧は右12mmHg，左13mmHg，細隙灯顕微鏡検査で両眼性の角膜浮腫を認め，虹彩上に人工虹彩が挿入されていた。角膜内皮細胞密度は両眼ともに測定不能であった。前眼部光干渉断層計では中心角膜厚は右698μm，左747μm，前房深度は右3.23mm，左3.17mmで，人工虹彩が角膜内皮に接触していたことから，人工虹彩による両眼性の水疱性角膜症と診断した。両眼に対して人工虹彩摘出術および角膜内皮移植術（DSAEK）を施行し，術後矯正視力は右0.6，左0.4と改善を認めた。

結論：整容目的での人工虹彩挿入後に発症した両眼性の水疱性角膜症を経験した。前房留置型人工虹彩による物理的な組織障害と慢性炎症により角膜内皮細胞の減少を生じたと考える。安易な海外での整容的治療には不可逆的な合併症を引き起こす可能性がある。

Abstract　Purpose：Various complications of artificial iris transplantation for cosmetic purposes have been reported, including iritis, corneal endothelial damage, cataracts, and secondary glaucoma. We describe a case of bilateral bullous keratopathy after insertion of an artificial iris for cosmetic purposes in India.

Case：The patient was a 46-year-old man who underwent bilateral artificial iris implantation in India in 2017. In early December 2020, he began to experience foggy vision, decreased visual acuity, and eye pain, after which he visited his previous doctor. Corrected visual acuity was 0.2 in the right eye and 0.03 in the left eye；intraocular pressure was 12 mmHg in the right eye and 13 mmHg in the left eye. Slit lamp examination revealed corneal edemas in both eyes and an artificial iris on the iris. Corneal endothelial cell density was not measurable. Anterior segment optical coherence tomography showed a corneal thickness of 698 μm in the right eye and 747 μm in the left eye, and an anterior chamber depth of 3.23 mm in the right eye and 3.17 mm in the left eye, and revealed the possibility that the artificial iris was in contact with the corneal endothelium. The artificial iris was removed and DSAEK was performed to treat the bullous keratopathy in both eyes caused by the artificial iris. Post-operative corrected visual acuity improved to 0.6 in the right eye and 0.4 in the left eye.

Conclusion：We experienced a case of bilateral bullous keratopathy caused by artificial iris transplantation for cosmetic purposes. Chronic physical tissue damage and inflammation caused by the artificial iris appear to have caused corneal endothelial cell loss. This case suggested that cosmetic artificial iris cause irreversible complications.
