栃木県内で経験した東洋眼虫症の1例

要約　背景：東洋眼虫Thelazia callipaedaは，ヒトと野生の哺乳類を宿主とする人畜共通の寄生虫であり，涙囊または結膜囊に寄生する。中間宿主はショウジョウバエ科のメマトイで，涙液や眼脂を舐めるときに幼虫を摂取する。東洋眼虫による眼感染は，日本では九州や西日本などの温暖な地域に多かったが，近年では北上する傾向がある。目的：栃木県内で発症した東洋眼虫による結膜感染例の報告。症例：栃木県に住む63歳男性が1か月前からの左眼の霧視と異物感で紹介受診した。自覚症状が生じる2週間前に，公園でハエに顔面周囲を執拗にまとわりつかれた。前医で虫体が左眼結膜に1隻発見され，除去された。患者の趣味は昆虫の写真撮影であった。所見：摘出された虫体は，その形態的特徴から東洋眼虫の雄と判定された。虫体の摘出後，自覚症状は消失した。結論：栃木県で東洋眼虫が発見されたことは，従来の報告にはない。温暖化のためにメマトイの活動期間が長くなったことがその理由であると推定される。

Abstract.　Background：Thelazia callipaeda are nematodes closely related to filaria. They infest animals with lacrimal and conjunctival sac as the habitat. The infection is transmitted to humans by flies. Ocular infestation in humans has been mainly reported in western Japan, and has been spreading to the north due to global warming. Purpose：To report conjunctival infestation by Thelazia callipaeda in a resident in Tochigi Prefecture. Case：A 63-year-old male was referred to us for blurring and foreign body sensation in the left eye since one month before. He was a resident in Tochigi prefecture, north of Tokyo. He had been plagued by a swarm of flies 2 weeks earlier. A worm had been removed from the left eye by the referring ophthalmologist. Findings：The removed worm was identified as a male Thelazia callipaeda by one of us specialized in parasitology. Subjective complaints in the left eye disappeared after removal of the worm. Conclusion：This case is the first reported one in Tochigi prefecture. Global warming appears to have resulted in the spread of habits of vector flies.
