多彩な臨床経過を呈した先天性網膜動静脈吻合の1例

A case of congenital arteriovenous communications with multiple complications

　先天性網膜動静脈吻合に多彩な出血を合併し，光凝固により著明な眼底所見の改善をみた50歳男子を報告する。右眼視力障害が突発した。眼底には黄斑部に網膜下を主体とする出血があり，乳頭から下耳側へ向かう網膜の動静脈は著しい拡張と蛇行を呈し，周辺部で多数の動静脈吻合を形成していた。出血は一時硝子体へも拡散したのち，自然に吸収され，病状は安定していた。2年8か月後，網膜中心静脈閉塞症が併発した。これに対し，キセノン光凝固で吻合血管を直接に凝固することにより，網膜出血の消褪と吻合血管の狭細化と視力の安定化が得られた。術後の螢光眼底造影で，動静脈吻合の閉塞が確認された。

A 50-year-old male presented with acute visual loss in his right eye. Ophthalmoscopy showed su-bretinal, retinal and preretinal hemorrhages in the macular area. Retinal vessels showed marked dila-tation and tortuosity with arteriovenous loop for-mations. Vitreous hemorrhage occurre but was absorbed spontaneously. After 32 months, central retinal vein occlusion developed, presumably secon-dary to decompensation by arteriovenous communi-cations. Xenon photocoagulation applied to the communicating vessels resulted in disappearance of retinal hemorrhages and attenuation of arter-iovenous communications. Fluorescein angiography showed complete obliteration of the communicat-ing vessels.
