網膜芽細胞腫に対するヘマトポルフィリン誘導体を用いた光化学療法

Photochemotherapy for retinoblastoma using hematoporphyrin derivative

　網膜芽細胞腫に対する新しい視力保存療法として，ヘマトポルフィリン誘導体を用いた光化学療法を6症例に行った。ヘマトポルフィリン誘導体を光照射の3日前に静注し，光源としてはアルゴンレーザーを用い，直径は2,000μm,40〜300mW，連続10分間照射した。

　この光化学療法の効果をカラー眼底写真，螢光眼底写真や病理組織学的に検討し，本療法は網膜芽細胞腫に対し有効であると判断した。今回は顕著な副作用はなかったが，過剰照射の問題などを含め，長期間の経過観察が必要であると思われた。

We treated 6 patients with retinoblastoma after a photochemotherapy principle using hematopor-phyrin derivative administration followed, 3 days later, by argon laser photoradiation with 488 and 514.5 nm wavelengths. The treatment proved to be effective in general.

We recommed the following guidelines for treat-ment of retinoblastoma with photochemotherapy. The hematoporphyrin is administered intravenously at the dosis of 2.5mg/kg. For argon laser photoradi-ation, 2-mm spot size is to be used at an intensity of 100mW for 10 minutes.
